Рецидив полипов при хроническом полипозном риносинусите после хирургического лечения остается многофакторной проблемой, на которую влияют эндотип хронического полипозного риносинусита, сопутствующие заболевания пациентов, хирургические методы. Частота рецидивов после хирургического лечения остается высокой, что увеличивает риск необходимости повторного хирургического вмешательства. В статье представлены результаты исследования, посвященного оценке эффективности применения биомедицинского клеточного продукта на основе мезенхимальных стволовых клеток обонятельной выстилки для увеличения длительности безрецидивного периода (отсутствия потребности в операции) у пациентов с хроническим полипозным риносинуситом. В исследование были включены 50 пациентов, разделенных на три группы: две основные, получившие клеточную терапию в разные сроки относительно хирургического лечения, и контрольная группа, получавшая только стандартное лечение. Анализ проводился с использованием методов Каплана-Майера и регрессии Кокса для оценки времени до повторного хирургического вмешательства в течение 3-летнего периода наблюдения. Результаты продемонстрировали статистически значимое увеличение безрецидивного периода в объединенной основной группе по сравнению с контролем (pлог-ранг = 0,023). Вероятность первой повторной операции в основной группе была в 5,3 раза ниже, чем в контрольной. Кроме того, применение клеточной терапии снизило риск выполнения второй повторной операции как минимум в 1,1 раза. Моделирование функции рисков показало, что риск рецидивов в контрольной группе начинал возрастать уже через год после операции, тогда как в основной группе он оставался стабильно низким на протяжении всего срока наблюдения.

Recurrence of polyps in chronic rhinosinusitis with nasal polyps after surgical treatment remains a multifactorial issue, influenced by the endotype of chronic rhinosinusitis with nasal polyps, patient comorbidities, surgical techniques, and regional treatment paradigms. The recurrence rate after surgical treatment remains high, which increases the risk of requiring repeated surgical intervention. This article presents the results of a study evaluating the effectiveness of a biomedical cell product based on olfactory mucosa mesenchymal stem cells in extending the relapse-free period (absence of need for surgery) in patients with chronic rhinosinusitis with nasal polyps. The study included 50 patients divided into three groups: two main groups receiving cell therapy at different times relative to surgical treatment, and a control group receiving only standard treatment. Analysis was performed using Kaplan-Meier methods and Cox regression to assess the time to repeated surgical intervention over a 3-year follow-up period. The results demonstrated a statistically significant extension of the relapse-free period in the combined main group compared to the control (p = 0.023). The probability of a first repeated surgery in the main group was 5.3 times lower than in the control group. The probability of a second repeated surgery in the main group was 1.1 times lower than in the control group. Modeling showed that relapse risk in the control group began to increase within one year post-surgery, whereas it remained stably low in the main group throughout the entire follow-up period.
