Задние экстрамуральные клетки решетчатого лабиринта, в частности клиновидно-решетчатые клетки (клетки Оноди), представляют собой важный объект изучения в современной оториноларингологии ввиду их тесной связи с критическими нейрососудистыми структурами основания черепа и высокого риска интраоперационных осложнений.</p></sec><sec><title>Цель</title><p>Цель. Систематизировать современные представления о вариантах строения, классификации и клиническом значении клиновидно-решетчатых клеток на основе данных конусно-лучевой компьютерной томографии (КЛКТ).</p><p>Объект и методы исследования. Проведен анализ литературных данных, включая оригинальные исследования, систематические обзоры и мета-анализы, найденные в базах данных PubMed, Google Scholar, CyberLeninka. Критериями поиска служили ключевые слова: «клетка Оноди», «клиновидно-решетчатая клетка», «конусно-лучевая компьютерная томография» на русском и английском языках. Особое внимание уделялось работам, опубликованным за последние 10 лет, с использованием методов КЛКТ-визуализации.</p></sec><sec><title>Результаты</title><p>Результаты. Обзор демонстрирует значительное разнообразие вариантов экстрамурального распространения задних решетчатых клеток, описанных в литературе. На основе их систематизации и с учетом собственных данных КЛКТ предложена модифицированная классификация клеток Оноди, включающая пять типов: I – верхний, II – верхнелатеральный, III – латеральный, IV – задне-верхний, V – интерсфеноидальный (межклиновидный).</p></sec><sec><title>Заключение</title><p>Заключение. Знание анатомических вариантов клиновидно-решетчатых клеток имеет важное значение для обеспечения безопасности эндоскопических вмешательств. КЛКТ-визуализация позволяет детально оценить эти воздухоносные полости и их отношение к критически важным нейрососудистым структурам. Тщательная предоперационная оценка с учетом существующих классификаций должна стать стандартом планирования операций в области клиновидной пазухи и задних отделов решетчатого лабиринта для предотвращения ятрогенных повреждений.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Posterior extramural ethmoid air cells, particularly sphenoethmoidal cells (Onodi cells), are an important object of study in modern othorhinolaryngology due to their close relationship with critical neurovascular structures of the skull base and the high risk of intraoperative complications.</p></sec><sec><title>Aim</title><p>Aim. To systematize current data on the variants of structure, classification, and clinical significance of sphenoethmoidal cells based on cone-beam computed tomography (CBCT) data.</p></sec><sec><title>Objects and methods</title><p>Objects and methods. An analysis of literary data was conducted, including original studies, systematic reviews, and meta-analyses found in PubMed, Google Scholar, and CyberLeninka databases. The search was performed using the following keywords in both English and Russian: «Onodi cell», «sphenoethmoidal cell» and «cone-beam computed tomography». Special attention was paid to works published in the last 10 years that used CBCT imaging methods.</p></sec><sec><title>Results</title><p>Results. The review demonstrates a significant diversity of variants of extramural extension of posterior ethmoid cells. Based on CBCT data, a modified classification of Onodi cells is proposed and detailed, including five types (I – superior, II – superolateral, III – lateral, IV – superoposterior, V – intersphenoidal).</p></sec><sec><title>Conclusion</title><p>Conclusion. Knowledge of anatomical variations of sphenoethmoidal cells is of great importance for ensuring the safety of endoscopic interventions. CBCT imaging allows for a detailed assessment of these air cells and their relationship to critical anatomical neurovascular structures.
