Обследовано 348 доношенных новорожденных. Выделено 3 группы крупновесных детей с учетом наличия у матери сахарного диабета 1 типа (Гр1), с избыточной массы или ожирения (Гр3) или отсутствия нарушений углеводного и жирового обмена (Гр2). Группа 4 (Гр4) – нормовесные от матерей без изменений углеводно­липидного обмена. Содержание адипоцитокинов, липидов, глюкозы и витамина Д исследованы в диадах «мать­дитя», сыворотке крови детей в динамике неонатального периода. Выявлены адипоцитокиновый дисбаланс и гиперлипидемия у матерей обследованных групп крупновесных детей, более выраженные при избыточной массе тела и ожирении. Отношение ТГ/ЛПВП было достоверно выше в Гр3. Установлено влияние метаболических нарушений у женщин на показатели физического развития, параметры гормонального статуса и липидограммы детей. Уровни лептина при рождении были значимо выше у крупновесных детей всех трех групп по сравнению с нормовесными (р &lt; 0,001, р = 0,002, р &lt; 0,001). В динамике раннего неонатального периода наблюдалось статистически значимое снижение содержания лептина на фоне повышения общего холестерина сыворотки. Уровни ЛПВП матерей, лептина и ИФР­1 пуповинной крови были ассоциированы с размером новорожденных. Выявленные перинатальные гормонально­метаболические закономерности оказывают выраженное влияние не только на состояние и антропометрический статус крупновесных детей, но и программируют темпы физического развития в периоде новорожденности.</p></abstract><trans-abstract xml:lang="en"><p>The purpose of the study was to analyze the characteristics of the adipocytokine status and lipid spectrum in large for gestational age (LGA) in the dynamics of the neonatal period, to assess the influence of the metabolic status of mothers on the analyzed indicators of their infants. 348 full­term newborns were examined. 3 groups of LGA were distinguished, taking into account the presence of type 1 diabetes mellitus in the mother (Gr1), overweight or obesity (Gr3) or the absence of disorders of carbohydrate and fat metabolism (Gr2). Group 4 (Gr4) – normal weight from mothers without changes in carbohydrate­lipid metabolism. The content of adipocytokines, lipids, glucose and vitamin D was studied in the “mother­child” dyads, blood serum of children in the dynamics of the neonatal period. An adipocytokine imbalance and hyperlipidemia were revealed in mothers of the examined groups of LGA, more pronounced with overweight and obesity. The TG/HDL ratio was significantly higher in Gr3. The influence of metabolic disorders in women on indicators of physical development, parameters of hormonal status and lipid profiles of children was established. Leptin levels at birth were significantly higher in LGA of all three groups compared to normal­weight children (p &lt; 0.001, p = 0.002, p &lt; 0.001). In the dynamics of the early neonatal period, a statistically significant decrease in leptin was observed against the background of an increase in total serum cholesterol. Maternal HDL, leptin, and cord blood IGF­1 levels were associated with neonatal size.
