Острым средним отитом к 3 годам переболевает каждый второй ребенок, к 5 годам – до 70 % детей. Хронический экссудативный средний отит обнаруживается при массовых осмотрах детей 2 – 7 лет в 30,2 % случаев. Пусковым моментом в развитии экссудативного среднего отита, как острой, так и хронической формы, является нарушение вентиляционной и дренажной функций слуховой трубы. Элиминация экссудата из барабанной полости после санации носоглотки и восстановления функции слуховой трубы происходит в 75 % случаев. В случаях, когда функция слуховой трубы не восстанавливается после аденотомии и консервативного лечения, показано выполнение шунтирования барабанной полости. В ряде стран методика тимпаностомии (шунтирования барабанной полости) широко внедрена как метод хирургического лечения при остром и хроническом экссудативном среднем отите. Однако единого мнения о методике выполнения вмешательства и дальнейшем ведении пациентов у специалистов нет. В статье дан обзор мнений и результатов использования методики шунтирования барабанной полости при экссудативном среднем отите у детей.</p></abstract><trans-abstract xml:lang="en"><p>Acute and chronic otitis media remain the leading causes of conductive hearing loss. Every second child by the age of 3 years and by the age of 5 years up to 70 % of children had acute otitis media. Chronic otitis media with effusion is detected during mass examinations of children aged from 2 to 7 years in 30.2 % of cases. The starting point in the development of otitis media with effusion both acute and chronic forms is a violation of the ventilation and drainage functions of the auditory tube. Elimination of effusion from the tympanic cavity after nasopharynx therapy and rehabilitation of the function of the auditory tube occurs in 75 % of cases. Tympanostomy is indicated where the function of the auditory tube is not restored after adenotomy or conservative treatment. Tympanostomy is widely introduced as a method of surgical treatment for otitis media with effusion both acute and chronic forms in a number of countries. However, there is no consensus on the method of intervention and further management of patients among specialists.
